A Guide for finding all of those pesky name tags (especially in Chapter 5), and the requirements for all endings in each chapter.
Note that I won’t be going over plot points and will only briefly mention them if needed. Keep an eye out for spoilers anyways.
After the long introduction of the setting and characters, we finally take control of Naomi (I guess Seiko is there but we don’t really control her a whole lot).
This Chapter has a total of four endings and six name tags. As far as I know, previous versions such as the PSP and PC ports only had three.
Leaving 1-A , you will have to go back in through the other door to get the loose board. From there you can choose to go either down or up. Going down just has additional character interaction (I don’t know if its actually required to progress the story).
Anyways, going up and bridging the gap, the duo finds themselves in cla*s 2-A when they encounter a dead body and meet a g-g-g-ghost.
After the sequence, the pair have to go back in. Here is where we find our first name tag.
Name Tag #1 – Yoshiyuki Shinohara (No relation to Seiko)
Simply examine his bones and you got a name tag (After getting all name tags you don’t get the in game pop up which is annoying).
Name Tag #2 – Yuzuha Mikami
Now that we have made it to the first floor, we will immediately see a skeleton and a note. The note has some nice additional background for the corpses that once attended Seisei Academy for Girls.
Name Tag #3 – Koito Fujiwara
Moving through the first floor hallway, you’ll come to a T-section, going left will lead us to poor Fujiwara-san and another letter that highlight how herself and her friends bit the dust.
Name Tag #4 – Asuka Kato
So from my understanding, Kato-san’s body spawns if you explore the first floor first. I read the other notes on the first floor and everything was fine but it seems going to the third floor before going to the first floor stops Kato-san from spawning in.
Anyways, Kato-san is found in Cla*sroom 5-A, along with another note.
Name Tag #5 – Sasha Shinonome
Our last victim from Seisei Academy for Girls. Like Kato-san, her body wont spawn if you explore the third floor first. From my understanding, you must explore the first floor first (and possibly read each corpse’s note as well.
Name Tag #6 – Shin Aichi
After collecting a key (found in the stairway by Cla*sroom 1-A). The duo finds themselves in cla*s 3-A. After reading the newspaper and having the events play out, examine the skeleton to get the name tag.
Bad End #1 – Curiosity got the better of you.
The first ending involves reading all five of the victims memoirs. This ending was first introduced in the 3DS version of the game.
Anyways the first memoir can be found in the lower half of Cla*sroom 1-A
Immediately after the second memoir can be found in cla*s 2-A.
And immediately after that we can find the third memoir tucked away in a corner at the end of the hall.
After some exploring, we finally get to the first floor. The fourth memoir can be found in the lower half of Cla*sroom 5-A.
Coming up to the third floor washrooms (this would have required you getting the board from the infirmary hallway, we find the fifth and final memoir just outside the boys’ room. After reading it, the screen will turn dark and the ending will play out.
Bad End #2 – Hello Nurse!
This one is quite simple to get. After exploring the third floor, Naomi and Seiko head to the infirmary. After a long scene, Naomi will get attacked by a ghost. Simply let your HP drop to zero and the ending will play out.
Bad End #3 – Daily Dose of Iron
This ending is probably one many get first before figuring out what you had to do. In order to get this ending, DO NOT obtain the key from the stairway near Cla*sroom 1-A. Escape the infirmary and let the scene play out. Here, similar events will play out like in the true ending however, Naomi is attacked by ghosts. I’ll leave it at that since the events following is pretty gruesome.

True End – This is Really Happening
In order to get this ending, you must have:
- Obtained the key from the stairway near Cla*sroom 1-A.
- Read the Newspaper in Cla*sroom 3-A.
- Investigate the stalls in the girl’s washroom.
- Escape the infirmary.
In doing so you will get the true ending. Ill spare the details as its pretty tragic.
And that’s chapter 1. Hope that clears up how to get 100% for this chapter. This is the easiest to get since there’s so many more name tags and endings in the next four chapters.
Well, the situation has quickly went from zero to a hundred in a heart beat. Ill skip over the Ms. Yui section as it doesn’t really get us anything except for the powerstone, which is a nice get out of jail free card but not really required. I would also recommend using the temp save often as there are many endings that happen back to back.
Chapter 2 has a total of six name tags and five endings. In terms of collecting them, most are along the path so they are pretty simple to get.
Once we take control of Kishinuma-kun, we make our way to the first floor where we will already find four out of the 6 name tags.
Name Tag #1 – Kai Shimada
Name Tag #2 – Tomohiro Ohkawa
Name Tag #3 – Masato Fu kuroi (Steam censors the name, haha).
Name Tag #4 – Hikari Kirigami
Since they are all in the same area, here’s one big group photo.
Name Tag #5 – Emi Kudou
Moving along the hallway and going left, we will find the entrance to Cla*sroom 4-A open, along with the first page of the victim’s memoir. More importantly is another corpse. Examine it a few times to get the name tag.
Name Tag #6 – Ryosuke Katayama
The last name tag of the chapter and giving us a hint to the overall fate of the Byakudan Senior High School students.
Play the game as normal and eventually you’ll reach the stairway leading to the third floor bathrooms. Hes tucked away in the corner.
Bad End #1 – Dinner is served
Like chapter 1, chapter 2 also has victim Memoirs. Ill spare the details of each one but here are the locations in order (has to be otherwise the next one won’t spawn).
The first memoir can be found along side Kudou-san’s corpse.
After reading the first memoir, head back to the entrance way where you’ll find the second memoir
After that you’ll have to make your way back to Cla*sroom 1-A. If you have progressed through the story, the backroom should be open. In there sits the third memoir.
Next, you will need to head back to the third floor bathroom hallway. upon entering the area you’ll see the fourth memoir I recommend getting them as you progress through the story otherwise you’ll do a lot of backtracking.
Lastly, you will need to head back to the second floor hallway that you started the chapter in. Going up, you’ll see the fifth and final memoir on the ground between Cla*sroom 2-A and the stairway. After reading it, you get treated to one of the cruellest bad ends.
Bad End #2 – Six Feet Under
Upon entering Cla*sroom 4-A (I think that’s the name), you’ll find a child spirit at the end of the room. Simply look at him to trigger the bad end. If you acquired the powerstone, you’ll have to look at him twice. There are many instances that you can get this bad end but its better to just get it out of the way now.
Bad End #3 – Splitting Up is a Bad Idea
After finishing up in 4-A, Kishinuma-kun and Ayumi will be momentarily separated. Rather than follow her, activate both levers (one in Cla*sroom 1-A and another in 4-A). Once the second lever is activated, walk down past the four corpses.
Bad End #4 – Flashbacks
After checking out the girl’s bathroom, head for the stairs and watch (or skip) the flashback. After it ends, you get a rather gruesome bad end. Keep in mind if you have the powerstone you will NOT be able to get this ending.
True End – Things went from Bad to Worse.
If you have been progressing through the story, you will need to collect doll parts.
The first can be found by the custodian’s closet (red door) on the first floor. Meanwhile the second will be in the stairway by Cla*sroom 2-A.
This one is a lot more simple to get as you just progress naturally. From there head back to the Infirmary and let the events play out.
And that concludes chapter 2. Collecting every thankfully wasn’t a huge pain since they are close to each other. The temp save feature is also a godsend.
Chapter 3 – Name Tags
Welcome to Chapter 3.
In terms of collecting stuff, I recommend making a lot of temp saves as there will be some loading of previous saves to get certain endings and name tags. Anyways lets jump into it.
There are 5 endings in this chapter but a total of 16 name tags.
Once we take control of Satoshi, we first head into Cla*sroom 2-A. Here we will find two corpses.
Name Tag #1 – Takayuki Motomura
Examine the corpse once to trigger dialogue, examine it again to get the name tag.
Name Tag #2 – Hiroko Motomura
In the same room you’ll find another corpse and a note (pretty tragic).
Name Tag #3 – Mitsuharu Ohtaki
Leaving through the upper door of Cla*sroom 2-A, you’ll find the first victim memoir, reading it gets you the name tag.
Name Tag #4 – Mao Kashiwatani
After acquiring another board, we make our way down to the first floor where (oh boy), another skeleton appears along with a note.
Name Tag #5 – Megumi Sugii
Making our way to the east side of the second floor, we go up the stairway towards the bathrooms, in the stairway you’ll find the name tag.
Name Tag #6 – Mika Naruse
After some progression we gain access to the second wing of the school. Right at the door is another body to examine.
Name Tag #7 – Kaori Kimura
Upon entering the second wing, we start by heading left (west), towards the stairwell. At the bottom of the stairs, you’ll find the body and the forth page of the victim memoir (if you’ve been reading them).
Name Tag #8 – Ryoko Fujiwara
Halfway there! In the same stairwell, climb all the way to the top and you’ll find another skeleton to add to your ever growing collection.
Name Tag #9 – Nao Takahashi
If you’ve been going in a straight path, eventually you’ll find another stairwell (east stairwell, I think?). Anyways, as always examine the body.
Name Tag #10 – Akihiko Kagurazaka
When we get control of Kizami, we’ll need to make our way back to the first wing of the school. Heading north from where the ghost boy was (head towards 5-A), lift the cabinet and a little bit ahead you’ll see Kagurazaka-san. The low visibility can make it a bit tricky to see them.
Name Tag #11 – Yuuki Tsuruta
Name Tag #12 – Hatto Sudo
Walking further ahead past Kagurazaka-san, you’ll see both Tsuruta-san and Sudo-san.
Name Tag #13 – Emi Urabe
Heading right from the previously mentioned corpses, we’ll come across another that has recently pa*sed and met a terrifying end.
Name Tag #14 – Kanau Sachimura
Heading all the way left in the same area as the previously mentioned corpses, we come across another that is inaccessible due to a gap. For this we need another board which can be found in a cabinet in the second wing entrance. Once acquired, place it and get the name tag.
Name Tag #15 – Mitsuki Yamamoto
Returning to the second wing, go back up the left stairwell and enter the second floor. You’ll come across Yamamoto-san, who just recently met her end. Examine her a few times to get her name tag.
Name Tag #16 – Kain Hagiwara
Upon waking up as Satoshi in the staff lounge, you can find the corpse on the left side of the room. It should be noted that if you’ve read all the memoirs up till this point, the final page blocks access to the name tag. I would advise making a temp save before reading the forth memoir.
Chapter 3 – Endings
This section covers the endings for chapter 3 since there is a lot of content for this chapter.
I will also be doing this for chapter 4 and 5.
Bad Ending #1 – Curiosity got the better of you….again.
Once again we make it to another round of victim memoirs (thankfully the last set). That being said, the first memoir can be found past Cla*sroom 2-A (you’ll need to go through the cla*sroom). This also nabs us a name tag.
Upon reaching the first floor, head to the entrance way. You’ll find the second memoir
After some exploring we eventually come across a nail puller. Using it on the door to the boy’s bathroom allows us access to the third memoir
After some time, you’ll eventually make it to the second wing. Going left (west), you’ll get to the stairwell. At the bottom of it, you’ll find the fourth memoir and a body.
After some story progression, Satoshi finds himself inside the staff lounge. In the left side of the room you’ll find the fifth memoir. It should be noted that having the page appear prevents you from getting the last name tag of the chapter (the scented beads could prevent the bad end also, although I am not 100% sure).
Bad Ending #2 – Oh There Goes Gravity
Close to the main entrance, you’ll find ghost boy again looking to up his body count. For this bad end, simply look at him. Note that if you accept Yuka’s gift, or walk past him towards the east stairway, it locks you out of this bad ending.
Bad Ending #3 – Slime Time
Continuing to explore the second wing, you’ll eventually see a hall with some weird green goo on the floor. For this ending, simply walk in it till your HP reaches zero.
Bad Ending #4 – I got my eye on you.
After some progression, The siblings decide to go back to the walkway for Yuka to take care of some…business. When playing as Yuka on the walkway, simply get caught by the ghost.
When you wake up as Satoshi in the Staff Lounge. Simply collect the video tape and read the research paper. Both are at the bottom of the room. From there, try to leave the room and you’ll initiate some dialogue.
And that concludes chapter 3. A lot of back tracking for this one and some loading required. Chapter 4 is about the same. Hope this helps
Chapter 4 – Name Tags
Welcome to Chapter 4.
At this point in the game, we start to play as multiple characters which can be quite messy collection wise.
In this chapter there is a total of 16 name tags, just like the last chapter.
Anyways, lets jump into it.
Name Tag #1 – Yuuya Kumada
After some story progression, we are finally able to go downstairs. upon reaching the landing of the east stairway, you’ll see the first name tag.
Name Tag #2 – Kotowe Kuroyama
Name Tag #3 – Mari Rokuhara
Triggering the cut scene where Satoshi examines where the second wing would be, we briefly catch up with Kizami and Yuka. When entering the bathroom, examine the second and forth stall (honestly just examine all of them just in case).
Name Tag #4 – Seiko Tanaka
Upon leaving the bathroom, we then take control of Ayumi (remember what i said about multiple characters?). Anyways, reentering the first wing, we immediately stumble upon this corpse.
Name Tag #5 – Yuma Misaki
Name Tag #6 – Tomoe Mizuki
Next, head up the hall, you’ll come across a body and a note, examine both.
Name Tag #7 – Yohsuke Maebana
Just before entering the west stairway, we’ll come across another body (Oh God the bodies have flesh on them now).
Name Tag #8 – Hotaru Kanzaki
Name Tag #9 – Nayuki Minatogawa
Lots of pairs this chapter. Anyways, these two can be found in the backroom in Cla*sroom 1-A.
Name Tag #10 – Hidemasa Kojima

Regaining control on Satoshi, we start by making our way to Cla*sroom 4-A. Reach the T-section and make a left, you’ll see a body and a note, examine both.
Name Tag #12 – Takuya Arai
After some story progression, we take control once again of Kishinuma and gain access to the pool area. In the locker room, examine the spot Kishinuma is standing on to get the name tag.
Name Tag #13 – Mitsuki Kamijou
Name Tag #14 – Kanzuki Saito
Name Tag #15 – Hirune Tanaka
Name Tag #16 – Tsukasa Tsuruta
After safely, draining the pool, the last 4 bodies can be found. Two at the lower half and two at the upper half of the pool.
Not too bad in terms of collection as they are all found easily as you progress the story.
Chapter 4 – Endings
Since we are playing as multiple parties, the method of collecting endings is messy, with the first two requiring a bit of moving around.
Please note: One of the ending is a secret ending and the other requires you to fail an event, I might need to spoil what happens to get each one, but ill try not spoil the ending itself.
In this chapter, there is a total of six endings.
Ending #1: Don’t run with scissors
Ill be sure to put spoiler tags for events in this ending .
For this ending, Do NOT save Naomi in the bathroom. Pick up the cellphone, as this will save some time and prevent backtracking
Next, go to where the entrance to the second wing would have been to trigger a cut scene. From there head back to the infirmary and talk to Touka (I think that’s her name?).
Next, you’ll want to head to cla*sroom 5-A. From there, Satoshi will realise he needs help. Further down the hall is Seiko’s Cellphone, DO NOT PICK IT UP. I would also suggest making a temp save here.
From there, make your way back to the infirmary and select either dialogue option to trigger the bad end.
Ending #2: Secret ending
For this ending, follow the above steps, except this time, pick up Seiko’s phone.
From there, go back to the infirmary, choose either option, then leave the room.
Ending #3 – Ghost boy returns
Once we take control of Ayumi, head back up the east stairway and enter the infirmary (we’ll need to come here anyways for plot progression). Next, go to the beds, where Ghost boy will reappear. Simply look directly at him to trigger the ending.
Ending #4 – No Lifeguard on Duty
After some story progression, we arrive at the pool area. After you pick up the pump room key, simply let the timer reach zero. I would recommend making a temp save before picking up the pump room key.
Ending #5 – Blocked Drainage
Much like the previous ending, we pick up pump room key, once the timer starts, go to the pump room at the lower half of the pool. Drain the water and watch the events unfold.
In order to get this ending you must:
- Successfully save Naomi.
- Pick up Seiko’s cellphone.
- Find all tongues and appease all spirits.
Chapter 5 – Name Tags #1-21
We finally arrive to chapter 5. This one is a mess of backtracking with a lot of ground to cover.
Anyways, there are a ma*sive total of 40 name tags!
This first section covers the main building and second wing.
Name Tag #1 – Eriya Seto
Collected when playing as Yuka, Climbing up the second wing stairs, you’ll come across it on the landing.
Name Tag #2 – Ayana Misono
Name Tag #3 – Toshimi Shinomiya
Keep in mind, when you come back to the second wing later, you will NOT be able to obtain these name tags.
Name Tag #4 – Kokuhaku Akaboji
Continuing down the hall from the slime, examine the bust.
Name Tag #5 – Rio Kazane
Continuing towards the stairwell, head downstairs where you’ll find another skeleton.
Name Tag #6 – Kaya Kizaki
Entering the east hallway, enter the music room to find the body.
Name Tag #7 – Sunoko Tamura
Name Tag #8 – Hideki Tachibana
Name Tag #9 – Mai Kumasaki
Moving up from the music room, you’ll immediately see these three together.
Name Tag #10 – Mio Kanaki
Moving along up the hallway, being careful not to be cornered, we come across Kanaki-san.
Name Tag #11 – Kagemi Yano (bring on the hard beats).
After our terrifying encounter with Kizami, we now take control of Ayumi. Be sure to choose to go back otherwise you’ll be locked into a bad end. Anyways, this body can be found near the stairs upon exiting Cla*sroom 1-A.
Name Tag #12 – Shiho Hasegawa
After heading downstairs and a bit of progression, we’ll respawn right in front of this corpse.
Name Tag #13 – Kyosuke Himonya
Heading back down towards the entrance, we’ll come across another skeleton.
Name Tag #14 – Kensuke Kurosaki
While playing as Yuka, we weren’t able to collect Kurosaki’s name tag. Now playing as Satoshi, we be able to examine his body, while exploring the second wing.
Name Tag #15 – Yuuki Nishimiya
Returning to the first wing, moving along the hall, we come across this name tag.
Name Tag #16 – Tohko Kirisaki
Heading upstairs to the 2F east hallway, we’ll come across Tohko-san’s body. We’ll need to come up here anyways to pull a lever.
Name Tag #17 – Yuuya Kizami
Once we switch back to Ayumi and gain access to the science lab, you’ll see Kizami’s name tag right by the door.
Name Tag #18 – Kazunari Takayanagi
Entering the Science Lab, you’ll come across Takayanagi in the lower half of the room.
Name Tag #19 – Naho Saenoki
Found in the reference room but only after you collect all pieces of her notebook.
Name Tag #20 – Takuji Mizuno
After a bit of story progression we head down the hall towards the incinerator. Heading down from the science Lab, you’ll see the corpse.
Name Tag #21 – Makoto Nagareyama
Entering the L-shaped hallway heading towards, the incinerator, you’ll come across Nagareyama-san at the bend of the hall.
Chapter 5 – Name Tags #22-40
The rest of these name tags are found in the bomb shelter.
Name Tag #22 – Nana Ogasawara
Upon entering the dissection room, you’ll find three name tags, the first is Nana, tucked in the corner.
Name Tag #23 – Chihaya Yamase
Found in the bucket close to Nana.
Name Tag #24 – Nari Amatoya
Closing off the Musashigawa Girls’ Middle school in Amatoya-san in a bucket in the top right corner of the room.
Body Disposal Room
Since the game doesn’t tell you upon subsequent playthroughs whose name tag you’ve collected (aside from the name tag list, which doesn’t really help), the best I can do is give all the names found in the corpse disposal room in no particular order. There should be a total of 11 here.

Don’t worry about the ones in the pool since we cannot access them.
Since the last sequence is pretty tense, it can be easy to miss the last few name tags. You can only collect them after the ritual. I’d also recommend temp saving since we are on a timer. Here they are in order

Heading to the Bathrooms, examine the debris in the ground
Name Tag #37 – Yasuji Morita
Name Tag #38 – Tomoyuki Hamada
Upon getting to the area with the pillars (where you had to follow the path the blue flame took). Fall through the trap door and these two will be in the corner and on the stairs.
Name Tag #39 – Yasuhiko Nomura
This one can be a bit tricky to get, Essentially solving the first blue flame puzzle again, it will be on the left side table (if that makes any sense). If you are able to examine the table you will get the name tag.
Name Tag #40 – Tohru Fujimoto
At the beginning of the head room, examine the left side table.
And that is all the name tags, with a combined chapter total of 84 name tags. Hope these helped!
Chapter 5 – Endings
Chapter 5 has multiple paths to take for endings, the longest being the first bad end. Ill try to be more specific on the requirements for each as it can be easy to get mixed up.
In this chapter there are 9 endings.
Let’s jump into it.
Ending #1 – Everything Gone Wrong
So this bad end is by far the longest to get and there are a lot of steps so ill lay them out for you.
Step 1: Get caught by Kizami
Pretty self explanatory, once you take control of Yuka, simply get caught and that locks into the bad end. This also works if you refuse to go back to Heavenly Host when playing as Kishinuma.
Step 2: Finally, when taking control of Ayumi, simply head downstairs and collect Naomi’s name tag..
Since we are in a bad end, there’s really no collection required aside from a few quest items and name tags.
Step 3: When playing as Satoshi, collect the DVD from the second wing.
When you regain control on Ayumi, immediately use the star to switch back to Satoshi. From there, go to the second wing west stairwell, collect Morishige’s phone (not sure if this is required), then go back to the entrance way. Next, head back to the custodian closet to watch the tape and to access the second floor bathroom
Step 4: Still playing as Satoshi, Head to the east stairway, to activate the lever
From there, go back down to custodian closet and a cut scene will play. From there, you’ll play as Ayumi, head towards the incinerator. The final scene will then play concluding the bad end.
Ending #2 – Lab Safety
This one can be a bit easy to miss. Simply examine the blood outside the Science Lab, examine the anatomical model, and then get caught.
Ending #3 – Nope, Nope, Nope. Nope
Makes my skin crawl, anyways, when you fall into the bug pit, simply let the timer reach zero, I would temp save before this sequence.
Ending #4 – Prelude to Book of Shadows
For this ending, play the game normally, but do NOT collect all the pieces of Naho’s note book. I would save before collecting the final piece and temp save for the rest of the playthrough. From there, simply play the game as normal i.e., going through the bomb shelter.
This ending also happens regardless if you take Yuka to the Bathroom in the bomb shelter or not.
Ending #5 – Hold It In
For this ending, follow the true ending. However, when separated from Naomi, take Yuka to the bathroom in the bomb shelter. From there, play the game as normal, complete the appeas*ment and escape to the walkway. Keep in mind since Naomi isn’t there, you’ll have to chant 5 times instead of 6. Once you reach the walkway the ending will play out.
Ending #6 – The Darkening
After reuniting with Ayumi and Kishinuma, head back to where you separated with Naomi (where the blood stopped), and try to go straight down the long hallway.
Go this way.
Ending #7 – Wrong Items
For this ending, simply give the wrong items during the ritual. The items are the notebook and the tape. This is the best way to get it, although I am unsure if giving all the items nets the same result.
Ending #8 – Together Forever
During the appeas*ment ritual, say the chant too many or too little times. Alternatively, fail to escape the school.
- Do NOT caught by Kizami and agree to go back to Heavenly Host.
- Fine all pieces of Naho’s notebook
They can be found in the entrance way, the hall outside Cla*sroom 1-A, the hall heading to the west stairwell, inside the Science Lab and in Cla*sroom 1-A
- Confront Naho in the reference room BEFORE regrouping with Satoshi. This grants you the baby statue.
- Head back to the east hall and read the diary in the infirmary (this will require the marble and baby statue. upon reading it, collect the black cat plushie shortly after.
- While still playing as Satoshi, head back to the second wing, go to the principal’s office, collect Sachiko’s tongue then head down to the bomb shelter.
- OPTIONAL: head back to the reference room to trigger an event that provides more back story to Sachiko and her mother.
- Eventually, you’ll reach the bomb shelter, while exploring you’ll get separated from Naomi at a T-section. Do NOT take Yuka to the shelter washroom.
- Reunite with Ayumi and Kishinuma and head towards the final area.
- Offer the correct items when prompted to (black cat plushie and tongue).
- Chant the correct number of times (six times).
