This guide will show how to get all secret endings: Dolls, Farm and Woods. As of the writing of this guide, the 4th levels is not yet available.

Dolls:

After knocking her out, pick up the girl and bring it down to the boiler room.

If you put her into the boiler, she will burn up to death.

When you return to the vehicle, a secret cutscene plays.

Farm:

After knocking Karen unconscious, pick her up with the wheelbarrow up and take to the cemetery behind the house.

Place her in a coffin with the lid open and a beam should shine on her.

She will turn into an enormous chicken boss. You will have to keep hitting the frying-pan against her to defeat her.

Then return to your van and you will see a hidden cut scene.

Woods:

After defeating witch, take her back to the revival lake.

Dumping her in the water will cause a light to shine above.

A secret cutscene will appear when you return to the vehicle.
