Never really wanted to play on insanity, yet you wanna achievement? No problem.

Where is this game saved?
Right before conversation with Saren, just hop in and finish him.
What else should I know about the save?
You have spectra equipment, since I played this as new game +
Liara and Wrex have quite good equipment as well. Managed to took him down on frist try, mostly with assault rifle (Shepard is soldier).
Save contains 3 manual saves and some autosaves (just to be sure). Main save you want is 111.
